The following are the terms and conditions for participating in one of our classes:
- Eligibility: You must be at least 18 years old to participate in a CPR class. If you are under 18, you may be able to participate if you have written or verbal consent from a parent or guardian.
- Health: You must be in good physical health and able to perform the physical techniques required in a CPR class. If you have any physical or medical conditions that may affect your ability to participate, you must disclose these to us before the class.
- By accessing and viewing the training videos containing graphic footage, you acknowledge and accept the responsibility of mentally preparing yourself for potentially distressing content. If you have any concerns or are squeamish about such material, it is your duty to discuss these concerns with the instructor or consider alternative training methods.
- COVID-19 Disclaimer: By participating in a CPR class, you acknowledge that COVID-19 is a contagious disease and that there is a risk of exposure or infection while attending class. You agree to follow all safety protocols and guidelines established by us and local health authorities, including wearing a mask and practicing social distancing. You understand and agree that you are assuming the risk of exposure or infection, and that such exposure or infection may lead to personal injury, illness, permanent disability, or death. You release us and our instructors from any and all liability, claims, actions, damages, costs, or expenses of any kind that may arise from or relate to your participation in the class.
- Payment: Payment must be received in full before certifications are issued. We accept payment by credit card or company check.
- Refunds: There are absolutely no refunds after the certifications are issued.
- Certification: Upon successful completion of the class, you will receive a certification. You must pass all skills and knowledge checks in order to receive certification. If you have any questions or do not understand any part of the class, it is your responsibility to ask the instructor for clarification. I also agree that I passed the skills, written, and knowledge checks during class and was issued a card for it unless otherwise noted after the class. It is the participants responsibility to type their name in correct to the online roster. For a card re-issue there is a $20 fee.
- Liability: By participating in a CPR class, you understand and agree that you may be exposed to physical and emotional risks, and you assume all responsibility for such risks. You release us and our instructors from any and all liability, claims, actions, damages, costs, or expenses of any kind that may arise from or relate to your participation in the class. You acknowledge that you have been informed of the nature of the class and the physical techniques that will be taught. You understand that it is your responsibility to use the techniques taught in the class appropriately and in accordance with any local laws, regulations, and guidelines. You release the instructor and Indiana Safety and Health, Inc. from any and all liability, claims, actions, damages, costs, or expenses of any kind that may arise from or relate to your use of the techniques taught in the class. This release includes any liability, claims, actions, damages, costs, or expenses that may arise from any failure to perform the techniques correctly or any injury or harm that may result from the use of the techniques. You understand and agree that this disclaimer applies to all future use of the techniques taught in the class and that it cannot be modified or waived by any oral or written representations or agreements.
- Changes to these terms: We reserve the right to change these terms and conditions at any time. Any changes will be posted on our website and will become effective upon posting.
By signing up for a CPR class, you acknowledge that you have read and agree to these terms and conditions.
